IL Pathways to Health’s Evidence-Based Programs

IL Pathways to Health provides evidence-based health management programs through partnerships across Illinois.

Evidence-based programs are proven to promote health and prevent disease among adults with chronic health conditions. IL Pathways to Health programs have been developed using rigorous research and demonstrate reliable and consistently positive changes in important health-related outcomes among participants.

An ever-increasing number of people are living longer with chronic pain and/or multiple chronic conditions. Research has proven that people with chronic conditions can learn skills to effectively manage their illness and improve health outcomes. IL Pathways to Health offers several evidence-based classes to improve functional abilities and deal more effectively with symptoms.

Take Charge of Your Health: Chronic Conditions
This program is designed to help participants deal with chronic conditions. Topics include: techniques to deal with frustration, fatigue, pain and isolation, appropriate exercise for maintaining and improving strength, flexibility, and endurance, appropriate use of medications, communicating effectively with family, friends, and health professionals, nutrition, and how to evaluate new treatments. People with chronic conditions and their caregivers can participate in this program.

Take Charge of Your Diabetes
This program is designed to help people with type 2 diabetes, are pre-diabetic and caregivers of adults with diabetes learn how to live well. Topics include: techniques to deal with the symptoms of diabetes, fatigue, pain, hyper/hypoglycemia, stress, depression, anger, fear and frustration; appropriate exercise for maintaining and improving strength and endurance; healthy eating, appropriate use of medication; and working with healthcare providers.

Tomando Control de Su Salud
Tomando Control de Su Salud is a culturally adapted version of the Take Charge of Your Health in Spanish for anyone with ongoing health conditions and their caregivers.

Tomando Control de Su Diabetes
Tomando Control de Su Diabetes is a Spanish language version of Take Charge of Your Diabetes for anyone living with diabetes, anyone who is pre-diabetic and their caregivers.

Workplace Chronic Disease Self-Management
This program is an adaptation of the Take Charge of Your Health Program for working professionals. It contains similar content, but is rearranged into 1-hour segments for the workplace. The programs benefits workplace wellness by: increasing productivity, decreasing absenteeism, reducing healthcare costs, and improving overall health and well-being.

Take Charge of Your Pain
This class is designed for people who have a diagnosis of chronic pain, offering strategies for dealing with their symptoms. Topics include: techniques to deal with frustration, fatigue, isolation, and poor sleep, appropriate exercise for maintaining and improving strength, appropriate use of medications, communicating effectively with family, friends, and health professionals, nutrition, pacing activity and rest, and how to evaluate new treatments.

A Matter of Balance
A Matter of Balance emphasizes practical strategies to reduce fear of falling and increase activity levels. Participants learn to view falls and fear of falling as controllable, set realistic goals to increase activity, change their environment to reduce fall risk factors, and exercise to increase strength and balance.

Bingocize
Bingocize combines a bingo-like game with exercise and health education. The program has been shown to increase older adults’ functional fitness, health knowledge, and social engagement.

Tai Chi for Arthritis and Fall Prevention
Tai Chi is an enjoyable exercise that can improve balance, relieve pain, and improve health and ability to do things. It is a slow exercise. It is easy to learn and good for all ability levels. The essential principles of Tai Chi include mind and body integration, fluid movements, controlled breathing and mental concentration. Tai Chi is one of the most effective exercises for the health of mind and body. It helps people to relax and feel better.

Social Connect is a facilitated, small group gathering, which meets six times for an hour by video-conferencing or phone. It is evidence-informed and uses mindfulness-based stress reduction strategies. Social Connect gatherings include an opportunity for light exercise and stretching to calm the tension in participants’ bodies, techniques to manage stress and anxiety, and connection with peers.

Aging Mastery Program
The Aging Mastery Program (AMP) is a fun and engaging education and behavior change incentive program for aging well. The AMP core curriculum covers 10 topics. The classroom experience is a mix of didactic and interactive learning with an emphasis on peer-to-peer interaction. When they graduate from AMP, participants have set goals for positive actions in many aspects of their lives such as exercise, nutrition, finances, advance care planning, community engagement, and healthy relationships.

Powerful Tools for Caregivers
Powerful Tools for Caregivers (PTC) is an evidence-based, self-care education program for family caregivers. Originally developed for caregivers of adults with chronic conditions, Powerful Tools for Caregivers has expanded the program to include classes for parents of children with special needs. Below is a general summary of PTC:

  • Education program – not a support group
  • 6-week series of classes
  • Focus on caregiver self-care
  • Scripted curriculum
  • Train-the-Trainer model

Savvy Caregiver
The Savvy Caregiver Workshop is a six session training series for informal caregivers careing for an adult with Alzheimers Disease or other cognative disorders. For most family caregivers, caregiving itself is a new role, one for which training is needed, just as a person would receive training for any new job. The Savvy Caregiver Program helps caregivers better understand the changes their loved ones are experiencing, and how to best provide individualized care for their care recipients throughout the progression of Alzheimer’s or dementia.

Stress Busting for Family Caregivers
Stress Busting for Family Caregivers is a nine-week evidence based program that teaches caregivers many new skills, including information about the disease process, stress management techniques, and a variety of other content. The program is designed to improve the quality of life for family caregivers who provide care for persons with Alzheimer's disease and other related dementias.
