Illinois Pathways to Health is an initiative designed to connect people who are interested in bettering their health and well-being with programs that are proven to do just that.

We believe a sustainable, integrated evidence-based health promotion network will improve access to valuable community-based health education. The network will support clinical-community linkages bridging gaps in public health prevention strategies. Illinois Pathways to Health will connect nationally recognized evidence based interventions/programs known to enhance the physical and psychological well-being to participants throughout Illinois.

What are “evidence-based” programs?

Evidence-based programs are developed and verified by researchers to promote self-management of chronic health conditions (such as diabetes and chronic pain), prevent falls, and foster individual well-being. Illinois Pathway to Health offers evidence-based programs for:

  • Falls Prevention & Physical Activity Programs
  • Social Connect
  • Aging Mastery
  • Caregiver Support Programs
