Take Charge of Your Health: Chronic Conditions is designed to improve the self-management skills of people living with ongoing health issues. Conditions like arthritis, mental health conditions, chronic pain, cancer, or diabetes can cause those affected to lose physical conditioning and suffer health problems over many years. This program teaches new strategies that will give participants the confidence, motivation, and skills needed to manage living with a chronic health condition.

LEADER TRAINING INFORMATION This is a CDSMP Facilitator Training. Attendance at all 13 sessions is mandatory. Space is limited; priority will be given to participants who have a workshop scheduled at the time of registration. * NON-ILLINOIS RESIDENTS If you are interested in participating in this training but live outside the State of Illinois, please contact Jaime Pena at jaime.pena@ageoptions.org for training fee information. * Take Charge of Your Health workshop leaders are trained by two Certified Master Trainers. Master Trainers will also provide ongoing support, technical assistance, and oversight to ensure program fidelity. TRAINING DEPOSIT AgeOptions is currently funded through a grant from the Administration on Community Living to expand program reach of the Take Charge series programs. Through this funding, training is offered at no cost to leader candidates who will facilitate workshops in Illinois and sign a delivery partner agreement. Delivery Partner agreements outline the responsibility of the agency, to include facilitating workshops within program fidelity and the submission of required paperwork. In return, AgeOptions provides extension technical assistance and monetary compensation. A $100 deposit will be required for all persons attending Take Charge trainings offered either in English or Spanish. This deposit will be refunded to the individual or agency (whomever made the payment) upon completion of two workshops. This is to encourage newly trained leaders to facilitate a minimum of two workshops within the first year of training. LEADER CERTIFICATION REQUIREMENTS • Attend all 13 training sessions taught by two Master Trainers, 32 hours total • Successfully complete two practice teach assignments during training • Facilitate one Take Charge of Your Health six-week workshop within one year of training • All leaders must be associated with a licensed organization. If you are unsure of licensed organizations in your area, please contact AgeOptions for clarification. If you do not work under an organization, you may work under the AgeOptions program license. MAINTAINING CERTIFICATION To maintain your active status as a workshop leader, your organization must have a current Memorandum of Agreement signed with AgeOptions. The Self-Management Resource Center requires leaders to complete one workshop series per year to maintain certification. All workshops should be registered with Illinois Pathways to Health online system. The online registration system provides ongoing reports for AgeOptions to track workshops on quarterly and annual bases. Please make sure to use the site when registering any workshop you facilitate. Who should take this training and maintaining Active Leader Status This training is for peer leaders: - Who in the past year have been unable to facilitate community workshops to maintain their active Leader status. - Who perform satisfactorily and wish to review key facilitation strategies. - Should be active and well performing Leader. - Have not facilitated a community workshop this year and intend to use the refresher training in lieu of workshops to maintain certification. - Will not have used the refresher training option two years in a row. People with type 2 diabetes attend the class in groups of 12-16. Classes are facilitated from a highly detailed manual by two trained Leaders, one or both of whom are peer leaders with diabetes themselves. Participants will make weekly action plans, share experiences, and help each other solve problems they encounter in creating and carrying out their self-management program. Physicians, diabetes educators, dietitians, and other health professionals both at Stanford and in the community, have reviewed all materials in the class.

People with type 2 diabetes attend the class in groups of 12-16. Classes are facilitated from a highly detailed manual by two trained Leaders, one or both of whom are peer leaders with diabetes themselves. Participants will make weekly action plans, share experiences, and help each other solve problems they encounter in creating and carrying out their self-management program. Physicians, diabetes educators, dietitians, and other health professionals both at Stanford and in the community, have reviewed all materials in the class.

LEADER TRAINING INFORMATION This is a Take Charge of Your Diabetes Cross Training. Attendance at all sessions is mandatory. Space is limited; priority will be given to participants who have a workshop scheduled at the time of registration. Take Charge of Your Diabetes workshop leaders are trained by two Certified Master Trainers. Master Trainers will also provide ongoing support, technical assistance, and oversight to ensure program fidelity. ELIGIBLITY Completed a Take Charge of Your Health leader training; Leaders trained prior to January 2020 must have completed the Take Charge of Your Health 2020 Update Training, and your certificate must be current. TRAINING DEPOSIT AgeOptions is currently funded through a grant from the Administration on Community Living to expand program reach of the Take Charge series programs. Through this funding, training is offered at no cost to leader candidates who will facilitate workshops in Illinois and sign a delivery partner agreement. Delivery Partner agreements outline the responsibility of the agency, to include facilitating workshops within program fidelity and the submission of required paperwork. In return, AgeOptions provides extension technical assistance and monetary compensation. A $50 deposit will be required for all persons attending Take Charge of Your Diabetes training offered either in English or Spanish. This deposit will be refunded to the individual or agency (whomever made the payment) upon completion of two workshops. This is to encourage newly trained leaders to facilitate a minimum of two workshops within the first year of training. LEADER CERTIFICATION REQUIREMENTS • Attend all training sessions taught by two Master Trainers, 8 hours total • Successfully complete one practice teach assignment during training • Facilitate one Take Charge of Your Diabetes six-week workshop within one year of training • All leaders must be associated with a licensed organization. If you are unsure of licensed organizations in your area, please contact AgeOptions for clarification. If you do not work under an organization, you may work under the AgeOptions program license. MAINTAINING CERTIFICATION To maintain your active status as a workshop leader, your organization must have a current Memorandum of Agreement signed with AgeOptions. The Self-Management Resource Center requires leaders to complete one workshop series per year to maintain certification. All workshops should be registered with Illinois Pathways to Health online system. The online registration system provides ongoing reports for AgeOptions to track workshops on quarterly and annual bases. Please make sure to use the site when registering any workshop you facilitate.
