Partner with IL Pathways to Health (ILPTH) to expand access to evidence-based health promotion programs. As a Delivery Partner, your organization plays a crucial role in delivering proven health education directly to communities across Illinois.

Depending on the program, your organization may either work under ILPTH’s statewide license or use your own license (if applicable). Our team will work with you to determine the best partnership and licensing structure for your needs.

What is a Delivery Partner?

A Delivery Partner is an organization that collaborates with IL Pathways to Health (ILPTH) to deliver evidence-based health promotion programs. This includes:

✔ Facilitating workshops using your staff or volunteers.

✔ Ensuring quality program delivery by following ILPTH guidelines and maintaining program fidelity.

✔ Creating a supportive and engaging learning environment for participants.

Note: Your organization may already have certified facilitators, or you may need training. ILPTH offers training when available—or we can connect you with resources to get facilitators certified.

What’s Required to Be a Delivery Partner?

To partner with ILPTH, your organization must be able to:

✔ Have Trained Facilitators or Participate in Training: Some of your facilitators may already be certified, while others may need training. If your staff isn’t already certified, ILPTH may provide training (based on availability) or help connect you with external certification opportunities. Check our upcoming training schedule for availability.

✔ Ensure Quality Program Delivery: Ensure that facilitators follow the ILPTH guidelines and maintain program fidelity.

✔ Provide a Supportive Environment: Create an environment conducive to learning (e.g., private space, necessary resources, etc.).

We’ll work with you to determine the best setup for your organization and the programs you wish to offer.

How It Works

Joining as a Delivery Partner is straightforward. Here’s how it works:

1️⃣ Express Interest: Fill out our form or email us at: info@ilpathwaystohealth.org to connect with our team.
2️⃣ Choose a Program: We’ll help determine the best fit for your organization.
3️⃣ Training & Implementation: We’ll guide you through the setup process to launch your program. If training is needed, we’ll check if ILPTH offers training for your selected program. If not, we can connect you with resources to get your leaders certified. Some organizations may already have trained facilitators. You can also check our upcoming training schedule.
4️⃣ Deliver the Program: Host workshops at your organization or collaborate with local sites. ILPTH will provide ongoing support throughout the process.

A: No! At times we provide training for facilitators for select programs, or we can connect you with how to get your leaders certified.

Q: What is the cost to become a Delivery Partner?
A: The cost to your organization depends on the program you choose and if you need to get staff certified in the program.

Q: How much can my organization make from partnering with IL Pathways?
A: Depending on the program and the funding available, we can reimburse for each workshop completed.

Q: Can we deliver multiple programs?
A: Yes! Depending on the contract, you may be able to offer multiple programs, such as Falls Prevention or the Take Charge series.
