AgeOptions operates the Illinois Pathways to Health Community Care Hub for Illinois. AgeOptions offers a cost-effective contracting solution to any organization that wants to provide social care including HRSN screening, referrals, and health promotion programs on a local, regional, or statewide level to their patients/members. We have a track record of successful program implementation, facilitation, data collection, and HIPAA-compliant data management for health promotion programs, with over 15 years of service to the Suburban Cook County area and beyond. 

Our network consists of Area Agencies on Aging, Care Coordination Units, senior centers, public health departments and more. Unlike national organizations, we have local expertise to connect clients/members to accessible resources in their community. Use our programs to increase member retention through investment in access to health education that improves overall well-being of your community, reach new potential members through broad marketing and recruitment tactics, and share why your organization is uniquely positioned to serve their needs. Decrease expenses as your members resolve health-related social needs and learn skills to better manage their conditions, while increasing revenue as members better utilize their primary care providers for appropriate diagnosis.

AgeOptions developed the Illinois Pathways to Health network hub with healthcare providers and insurers in mind. By contracting with AgeOptions, healthcare providers and insurers can access a statewide network of service providers. This network of agencies includes area agencies on aging, care coordination units, departments of public health and others who have sterling reputations in their communities and either deliver or refer to social drivers of health (SDOH) services should they see other client needs.

  • Health-Related Social Needs Screening and Referrals: Our trained Community Health Workers utilize the CMS Accountable Communities HRSN screening to assess needs and resolve them. We utilize motivational interviewing techniques, cultural humility, and knowledge of local resources to resolve identified needs.
  • Member Engagement: Boost member retention and reach new members by offering health education that improves well-being and demonstrates your commitment to community health. Our workshops extend the care you provide, targeting chronic pain, disease management, and fall prevention.
  • Cost Savings: Empower members with the skills to better manage their conditions, leading to reduced healthcare costs and improved use of primary care services. Our evidence-based programs help you achieve care management goals and improve quality outcomes.
  • Oversight & Quality Assurance: AgeOptions provides oversight and leadership to the service providers for contract compliance and program quality and fidelity. We strive to ensure that every program is delivered consistently and effectively, upholding trust and excellence throughout the network.
  • Customized Outreach: Workshops can be co-branded with your organization’s logo on promotional materials, participant workbooks, and textbooks. We also offer opportunities for branded consumer goods (e.g., pedometers, hats, bags) as incentives for program completion, reinforcing your brand’s association with health promotion.
  • Technology Empowerment: Increase your network’s functionality with technology. Online program costs include time to get participants familiar with Zoom through one-on-one technical assistance. This increased knowledge and skill for participants may lead to increased ability to participate in telemedicine visits and utilize Zoom or other online programs to do other health promotion activities and/or engage with others socially. 
  • Comprehensive Participant Support: As an add on to any workshop, AgeOptions can also complete a social driver of health assessment screen on each registered participant prior to the start of the workshop and make direct referrals to address identified needs.

All the programs listed below are tailored for groups of 8-20 older adults and can be conducted both in-person or virtually—in English with select programs available in Spanish.

We offer these programs in convenient community locations, including senior centers, libraries, faith communities, and clinical settings like hospitals and doctor’s offices.

 

Increase your activity, build strength and improve balance to reduce your risk of falls. Learn ways to modify your environment and more to prevent falling.

A Matter of Balance

A Matter of Balance empowers older adults to manage fall risks and boost activity levels. Participants learn strategies to reduce the fear of falling while enhancing balance, flexibility, and strength, ultimately viewing falls as controllable challenges.

Workplace Chronic Disease Self-Management

Workplace Take Charge of Your Health is an interactive workshop for employed individuals with chronic physical or mental health conditions, such as arthritis, diabetes, heart disease, and depression. Participants will learn essential skills for managing their conditions, including work-life balance, exercise, healthy eating, symptom management, weight loss, and effective communication.

Diabetes Prevention Program

The Diabetes Prevention Program helps individuals prevent type 2 diabetes through a CDC-developed curriculum that fosters lasting lifestyle changes. Over one year, participants learn about healthy eating, physical activity, and stress management, all while receiving support from a lifestyle coach and peers.

Social Connect is a facilitated small group online gathering focused on gentle movements, stress reduction, and building connections. Participants learn techniques to boost well-being, improve sleep, increase compassion, and strengthen the immune system.
